A 38-year-old patient came into our Clifton office last spring. She'd had two kids, was back to her pre-pregnancy weight, and worked out four days a week. But her abdomen wouldn't tighten no matter what she did. Her OB-GYN had confirmed a 2-finger diastasis recti—the abdominal separation common after pregnancy. She wanted to know: could EMSculpt Neo actually help, or was it just another overhyped body contouring device?
It's a fair question. "Build muscle and burn fat without exercise" sounds like late-night infomercial territory. So let's look at what the research says, who this treatment actually works for, and where its limits are.
How EMSculpt Neo works
EMSculpt Neo is an FDA-cleared device that combines two mechanisms in a single applicator:
HIFEM (High-Intensity Focused Electromagnetic Energy) generates electromagnetic fields that trigger supramaximal muscle contractions—contractions stronger than anything your nervous system can produce voluntarily. During a 30-minute session, the target muscle group contracts roughly 20,000 times.
Radiofrequency (RF) heating runs simultaneously, raising subcutaneous fat temperature to the point of apoptosis (programmed cell death). The body clears these dead fat cells over the following weeks through the lymphatic system.
The original EMSculpt only had the HIFEM component. The Neo added RF, which is why the fat reduction numbers improved in later studies.
The research: what the numbers actually mean
The most-cited study comes from Jacob et al. (2021) in Lasers in Surgery and Medicine, which used MRI imaging—not before-and-after photos—to measure outcomes:
- 25% average increase in muscle volume
- 30% average reduction in subcutaneous fat thickness
- Results measured at 3 months post-treatment
A separate multi-center study published in the Journal of Cosmetic Dermatology (Kent & Kinney, 2021) found similar numbers across 77 patients, with 91% patient satisfaction at the 3-month mark.
But here's the context that marketing materials leave out:
- Study participants were relatively fit (BMI under 30)
- They maintained their normal diet and exercise during the trial
- "25% increase in muscle volume" doesn't mean you'll look 25% more muscular. On a person with average abdominal muscle mass, that translates to roughly a centimeter of added thickness across the rectus abdominis. Visible? Yes. Dramatic? Depends on your starting body fat percentage.
- Individual variation was wide. Some participants gained closer to 15%, others over 30%.
What 25% muscle gain looks like in practice
On a lean person (under 20% body fat), a 25% increase in abdominal muscle mass shows up as noticeably sharper definition—the lines between muscle segments become visible where they weren't before. On someone with moderate body fat (25-30%), the change feels more like a tighter, firmer midsection. You'll notice it when you press your hand against your stomach. Others might notice your clothes fit differently. You won't walk out looking like a fitness influencer.
For glutes, the change is more visible because the muscle group is larger. Patients typically describe a "lifted" appearance and firmer feel.
What EMSculpt Neo can do
Build real muscle. The supramaximal contractions trigger the same adaptive response as resistance training—muscle fibers thicken (hypertrophy) and new myofibrils form. You'll gain measurable muscle mass in the treated area.
Reduce localized fat. RF heating destroys fat cells permanently. Those specific cells don't regenerate. Most patients lose 1-2 inches in the treated area.
Treat multiple areas. Abdomen, buttocks, thighs, arms (biceps and triceps), and calves are all treatable.
What EMSculpt Neo cannot do
This section matters more than the one above.
It's not weight loss. Like other non-surgical treatment options, EMSculpt refines. It doesn't transform. If you need to lose 30 pounds, start with nutrition, exercise, or a medically supervised weight loss program. EMSculpt is for the gap between "healthy weight" and "the definition I want."
It won't create visible abs through a fat layer. EMSculpt can strengthen your rectus abdominis and reduce some overlying fat simultaneously. But visible abs require both muscle mass AND low body fat percentage (roughly 15-18% for women, 10-14% for men). If you're at 30% body fat, the muscle will be there—you just won't see it until the fat drops further.
It doesn't replace exercise. No cardiovascular benefit. No endorphin release. No flexibility improvement. No bone density gains. Think of it as one specific tool, not a fitness program.
Results fade without maintenance. Muscle built by EMSculpt follows the same rules as muscle built in a gym: use it or lose it. Plan for maintenance sessions every 3-6 months, or supplement with your own training.
Who we treat (and who we turn away)
In our practice, Dr. Patel screens every EMSculpt consultation personally. The patients who get the best results fall into a few categories:
- Post-pregnancy diastasis recti. This is one of the strongest use cases. The device can re-approximate separated abdominal muscles in a way that crunches alone often can't. Many post-pregnancy patients also pair this with PRP microneedling for skin texture improvement.
- Fit people who've plateaued. You work out regularly but can't get more definition in a specific area.
- Injury-limited patients. Shoulder surgery means you can't do overhead presses, but you still want arm definition.
- Buttock lifting without surgery. Glute results tend to be the most visually obvious.
We turn patients away when:
- BMI is significantly over 30. The research doesn't support good outcomes at higher body fat levels. We'll suggest starting with weight loss and revisiting EMSculpt later.
- Expectations are surgical. If someone wants the result that liposuction or a tummy tuck delivers, we say so. EMSculpt is a different category of result.
- Contraindications exist. Pacemakers, metal implants in the treatment zone, or pregnancy are absolute disqualifiers.
Dr. Patel's screening philosophy: "I'd rather lose a sale than have an unhappy patient. If EMSculpt isn't the right fit, I'll tell you what is."
Combining EMSculpt with diet and lifestyle
The patients who see the best results don't just do four sessions and stop. They use EMSculpt as an accelerant within a broader plan:
Protein intake matters. Your muscles need amino acids to build. Dr. Patel recommends patients aim for 0.7-1g of protein per pound of body weight during their treatment series. Without adequate protein, you're limiting how much muscle the contractions can actually build.
Reduce inflammatory foods during your treatment series. Excess sugar and alcohol increase systemic inflammation, which can blunt the fat apoptosis response.
Keep exercising. EMSculpt results stack on top of your existing fitness work. Patients who maintain a regular exercise routine see better outcomes than those who rely on the device alone.
Hydration. Your lymphatic system needs water to clear the dead fat cells. Simple, but patients who stay well-hydrated report less post-treatment soreness too.

The treatment experience
You lie down while the applicators are strapped to the treatment area. The device ramps up gradually—your first sensation is a deep, involuntary contraction. It's not painful, but it's strange. Your muscles are working hard and your brain didn't tell them to.
Most patients describe it as intense but tolerable. The first session is the most unfamiliar. By session three, you know what to expect.
- Duration: 30 minutes per area
- Frequency: 4 sessions over 2 weeks (standard protocol)
- Areas per session: 2-4 areas
- Downtime: None. Walk out and go about your day.
The RF component feels warm but not uncomfortable. Some patients feel mildly sore the next day—similar to a good workout.
Results timeline
Immediately after: Your muscles feel fatigued, like you've done an intense gym session.
Days 1-3: Possible mild soreness.
Weeks 2-4: Increased firmness becomes noticeable to touch.
Months 1-2: Visible changes start appearing. Clothes fit differently.
Month 3: Full results. Both fat reduction and muscle building are complete at this point.
Cost at Millennium Skin and Laser
- $300/session (treating 2-4 areas)
- Standard protocol: 4 sessions
- Package pricing available
For comparison:
- Liposuction runs $3,000-8,000 with surgical risks and weeks of recovery
- CoolSculpting costs similar per area but builds zero muscle
- A personal trainer costs $200-400/month indefinitely
For the right candidate, EMSculpt delivers measurable results at a reasonable price point. For the wrong candidate, it's $1,200 spent on modest improvements. That's why the screening consultation matters.
EMSculpt Neo vs. other body sculpting
vs. CoolSculpting: CoolSculpting freezes fat cells but does nothing for muscle. If you want fat reduction only, it's a reasonable alternative. If you want muscle definition, EMSculpt is the only non-surgical option that delivers both.
vs. Liposuction: Lipo removes dramatically more fat but involves anesthesia, incisions, compression garments, and 2-4 weeks of recovery. Different category entirely.
vs. Exercise alone: Exercise is cheaper, has more health benefits, and works forever. EMSculpt fills a specific gap—isolated muscle groups you can't target effectively, post-pregnancy repair, or pushing past a plateau.
Frequently asked questions
How many sessions do I need?
4 sessions over 2 weeks is standard. Some patients do 6 for more pronounced results. Maintenance every 3-6 months sustains the gains.
Does it hurt?
Intense, not painful. The involuntary contractions feel strange at first. Most patients adjust quickly.
Can I work out the same day?
Yes, though your muscles may be fatigued. Some patients exercise before their session to "pre-exhaust" the muscles.
How long do results last?
Destroyed fat cells are permanently gone. Muscle gains follow the same rules as gym-built muscle—maintain them with periodic sessions or your own training.
Is it safe?
EMSculpt Neo is FDA-cleared. Contraindications include pacemakers, metal implants in the treatment area, and pregnancy. We review your full health history before treatment.
Can men get EMSculpt?
Absolutely. A significant portion of our EMSculpt patients are men targeting abs, arms, or glutes.
