Here is something most people do not realize: the most effective serum for your skin is already inside your body. Your blood plasma contains over 1,100 proteins, including platelet-derived growth factor (PDGF), vascular endothelial growth factor (VEGF), and transforming growth factor-beta (TGF-B)—all of which directly signal your skin cells to produce collagen and repair tissue. No topical product you can buy at Sephora contains these growth factors in bioactive form.
That is the science behind PRP microneedling, sometimes called the "vampire facial." At Millennium Skin and Laser in Clifton, NJ, we offer PRP microneedling for skin rejuvenation, acne scarring, hair thinning, and overall skin health. Here is an honest look at the evidence and what you can realistically expect.
What Is PRP, and Why Does It Work?
PRP stands for Platelet-Rich Plasma. It is a concentrated portion of your own blood—typically 3 to 5 times the normal platelet concentration—that contains the growth factors and signaling proteins responsible for tissue healing.
A 2020 study in Stem Cells Translational Medicine (Gentile et al.) analyzed PRP's mechanism of action and confirmed that PRP releases at least seven key growth factors: PDGF, TGF-B, VEGF, EGF, FGF, IGF-1, and HGF. These proteins directly activate fibroblasts (the cells that manufacture collagen) and stimulate angiogenesis (new blood vessel formation), which improves nutrient delivery to tissue.
Because PRP comes from your own blood, there is zero risk of allergic reaction or rejection.
How We Prepare Your PRP
Dr. Patel draws the blood herself and oversees the centrifuge process to ensure quality at every step:
- We draw 1-2 tubes of blood from your arm (similar to a routine blood test)
- Blood goes into a centrifuge and spins at high speed to separate red blood cells, white blood cells, and plasma
- The platelet-rich layer is extracted—this golden liquid is your PRP
- PRP is applied to your skin during and after microneedling
What Is in PRP?
- Platelets — cells that trigger clotting and release growth factors upon activation
- PDGF, VEGF, TGF-B, EGF — proteins that directly stimulate collagen synthesis and cell turnover
- Cytokines — signaling molecules that coordinate the healing response
- Fibrin matrix — a scaffolding protein that helps growth factors stay concentrated at the treatment site
What Is Microneedling?
Microneedling (also called collagen induction therapy) uses tiny needles to create controlled micro-injuries in the skin. This triggers your body's wound healing cascade:
- Inflammation phase — skin recognizes injury and floods the area with healing cells
- Proliferation phase — fibroblasts begin producing new collagen and elastin
- Remodeling phase — collagen cross-links and strengthens over weeks to months
The result is firmer, smoother skin with improved texture.
PRP + Microneedling: What the Research Shows
Microneedling alone is effective. But adding PRP produces measurably better outcomes.
Alser and Goutos published a systematic review in the Journal of Cosmetic Dermatology (2020) analyzing 11 controlled studies comparing microneedling with PRP versus microneedling alone. Their conclusion: the combination produced statistically significant improvements in scar depth, skin texture, and patient satisfaction scores compared to microneedling alone. Specifically, acne scar patients saw a 30-40% greater improvement in Goodman and Baron scar grading when PRP was added.
Why the difference? The microchannels created by needles act as direct delivery pathways. Instead of PRP sitting on top of intact skin (where the stratum corneum blocks most absorption), the channels allow growth factors to reach the dermal layer where fibroblasts live. Think of microneedling as opening thousands of tiny doors, and PRP as delivering a repair crew through each one.
Research confirms that adding PRP to microneedling produces better outcomes across multiple skin concerns.
What Can PRP Microneedling Treat?
Acne Scars
This is the most well-studied use. The combination of collagen induction and concentrated growth factors helps:
- Fill in rolling and boxcar scars over time
- Smooth overall texture
- Reduce post-inflammatory discoloration
Multiple sessions are typically needed. The Alser 2020 review found that 4-6 sessions spaced 4 weeks apart produced the most consistent scar improvement. See our full guide to acne scar treatment options for a detailed comparison of all approaches.
Fine Lines and Wrinkles
Increased collagen production helps soften fine lines around the eyes and mouth, improve skin firmness, and restore a smoother texture. Patients in their 40s and 50s tend to see the most dramatic difference, since their baseline collagen production has slowed enough for the PRP boost to be noticeable.
Skin Texture and Tone
PRP microneedling improves rough, uneven texture, enlarged pores, dull skin, and mild sun damage. Many patients describe a "glass skin" effect after their third session—not from a filter, but from genuine collagen remodeling.
Hyperpigmentation
Can help fade dark spots from sun damage, post-inflammatory hyperpigmentation (PIH), and uneven tone. For melasma specifically, we use a conservative approach—microneedling can sometimes trigger more pigmentation in melasma-prone skin if depth settings are too aggressive.
Stretch Marks
PRP microneedling can improve stretch mark appearance, though results are more variable and multiple sessions are needed.

The Treatment Experience
Before Your Appointment
- Stop retinoids 3-5 days before
- Avoid blood thinners (including ibuprofen) for a few days if possible
- Come with a clean face, no makeup
- Stay well hydrated the day before—dehydration reduces the quality of the PRP draw
During Treatment (About 60-90 Minutes)
- Blood draw — Dr. Patel draws 1-2 tubes from your arm
- Numbing — topical lidocaine cream is applied for 20-30 minutes
- PRP preparation — while you are numbing, we spin your blood to extract PRP
- Microneedling — the device passes over your face, creating microchannels at 0.5-2.5mm depth depending on the area
- PRP application — PRP is applied and massaged into the channels
- Final layer — remaining PRP or a soothing hyaluronic acid serum is applied
What It Feels Like
With numbing cream, most patients describe a scratching or vibrating sensation. Mild discomfort, not painful. The areas around the eyes, nose, and forehead are more sensitive. Most patients rate it 3-4 out of 10.
Immediately After
Your face will be red, like a moderate sunburn. You will also notice some swelling (especially around the eyes), tight and warm skin, and tiny pinpoint bleeding (the origin of the "vampire facial" name).
Recovery and Downtime
Day 1-2: Redness and swelling. Stay home if you can. Skin feels tight and dry. Avoid makeup, skincare products, and sun.
Day 3-4: Redness fades significantly. May have some flaking. Can return to most activities. Light makeup is okay.
Day 5-7: Skin mostly back to normal. You may notice an initial glow. Resume normal skincare.
Full results timeline:
- 2-4 weeks: initial improvement visible
- 3-6 months: optimal results as collagen continues building
- 3-4 treatments recommended for best overall results
PRP for Hair Restoration
PRP is not only for skin. We also offer PRP injections for hair thinning and early-stage hair loss.
The mechanism is similar: growth factors from your platelets stimulate dormant hair follicles, increase blood supply to the follicle, and prolong the active growth phase (anagen) of the hair cycle. A 2019 meta-analysis in Dermatologic Surgery (Giordano et al.) reviewing 10 randomized controlled trials found that PRP injections produced a significant increase in hair density and thickness compared to placebo.
Dr. Patel injects PRP directly into the scalp in areas of thinning. The treatment takes about 30 minutes, with minimal discomfort (we use a numbing technique). Most patients need 3-4 monthly sessions followed by maintenance every 6-12 months.
PRP for hair works best for:
- Androgenetic alopecia (genetic thinning) caught early
- Women with diffuse thinning
- Patients who want a non-surgical option before considering transplants
It is less effective for patients with complete baldness in an area—the follicles need to be present (even if dormant) for PRP to reactivate them.
Who Is a Good Candidate for PRP Microneedling?
Ideal candidates:
- Acne scars (rolling, boxcar)
- Fine lines and early wrinkles
- Dull, textured skin
- Want natural improvement without injectables like fillers
- Willing to complete a series of sessions
- All skin types, including darker skin tones—microneedling does not target pigment
Not ideal candidates:
- Active acne or skin infections
- Blood disorders or currently on blood thinners
- History of keloid scarring
- Pregnant or breastfeeding
- Active rosacea or eczema flare
- Looking for the results of a facelift—see our guide to non-surgical facelift alternatives if that is what you are after
PRP Microneedling vs. Other Treatments
vs. Microneedling Alone
The Alser 2020 systematic review found that adding PRP produced 30-40% greater improvement in acne scarring outcomes. For patients investing in the time and aftercare of microneedling, adding PRP is worth the incremental cost.
vs. Laser Resurfacing
Laser can be more effective for pigmentation and deep wrinkles but has more downtime. Some lasers are not safe for darker skin types. Microneedling is safe across Fitzpatrick I-VI. Learn more about laser skin rejuvenation and collagen remodeling.
vs. Chemical Peels
Peels address surface-level concerns. Microneedling penetrates deeper for collagen stimulation. The two can be combined on separate visits for a more complete protocol.
vs. Fillers
Fillers provide immediate volume. PRP microneedling provides gradual improvement through your own collagen production. Different tools for different problems—and they can complement each other.
